    Job Description

    Under the co-supervision of the CMO and Director of Diabetes Programs, the Nutritionist is responsible for planning and conducting nutritional programs to assist in the promotion of health and control of disease processes of the patient population. Nutritionists must work to identify the source of nutrition issues for the facilities' populations and find ways to address them through programs to access healthy food. Nutritionists are responsible for assessments, establishing short and long-term health goals, monitoring and coordinating patients' nutritional treatment plans, performing education about health and nutrition, connecting them with health care providers, and evaluating their progress.

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    · Serves as a resource on nutrition and dietetics to staff nurses, providers, and care coordinators.

    · Develops policies for facilities that work towards reducing chronic disease by increasing access and consumption of healthy food/beverages, improving physical activity opportunities and linkages to critical community resources.

    · Provides nutrition and health assessment, education, and services to medically at-risk patients in GROUP and INDIVIDUAL trainings.

    · Counsels patients on any dietary, weight, and healthy eating habits.

    · Developing a budget appropriate nutrition plant for patients.

    · Assesses patients' nutritional and health needs.

    · Evaluates success of meal plans regarding patients' health.

    · Creates/utilizes educational material about healthy eating habits.

    · Continuing education as it contributes to the latest nutritional plans of care, including obtaining certification as Certified Lifestyle Coach for NDPP.

    · Assists with food demonstrations.

    · Speaks in group sessions of the Diabetes Prevention & Management Program.

    · Participates in community events and health fairs.

    · Schedules and contacts patients in a timely manner- within 3 business days of referral.

    · Documents appropriately as per training by Director. Health Care Partners Of South Carolina, Inc.

    · Conducts reminder calls and weekly follow-up calls as required.

    · Collaborates with Director of Diabetes Programs weekly.

    · Assists Program Director with tasks outside of scope of job, i.e., facility events.

    · Travels to other sites as needed.

    · Keeps inventory as appropriate with supplies and materials.

    · Reschedules appointments as necessary due to scheduling conflicts.

    · Works with translator/interpreter for non-English speaking patient scheduling.

    · Assists with researching and obtaining Grants.

    · Other duties as assigned.

    Requirements

    Current license to practice as a registered (preferred certified) Nutritionist in the state of SC.

    Bilingual preferred

    Two years' experience in a community setting

    Ability to provide effective nutrition counseling to diverse populations.

    Highly organized and task oriented, with ability to keep accurate notes and records.

    Experience with electronic medical records is desirable.

    Proficiency in communication technologies (email, cell phone, Microsoft Office Programs).

    Local knowledge about and connections to community health care and social welfare resources is desirable.

    Initiative-taker with the ability to work independently.

    Work occasional Saturdays.

    Must be punctual and have reliable transportation.

    Must be able to pass a background check.

    Must follow ALL HIPAA and Confidentiality policies and procedures.
