    As a Home Care LPN working in our Savannah, GA territory, you will work one-on-one with an individual in their home and assist them with tasks such as (but not limited to) assessments, vital signs, tube feedings, medication administration, and activities of daily living.

    Other responsibilities for this position include:

    • Adherence to client care plan/485
    • Care documentation, client observations and outcome evaluation
    • Client and family education
    • Working closely with Help at Home clinical team and client PCP as needed to address clients' medical needs

    Here is a snapshot of our clients who can use your help:

    • Statesboro - Monday-Sunday 5 flex hours/day. Seizure disorder, bed bound, CPAP, tube feedings
    • Georgetown - Monday-Sunday 8a-1p. Non-verbal, gait-belt transfers, ROM exercises
    • Midtown Savannah - Saturday & Sunday 7a-7p. Bed bound, tube feedings
    • Southside Savannah - Saturday & Sunday 7a-7p. Tube feedings, medication administration

    Preferred Skills/Abilities:

    • Excellent verbal and written communication skills
    • Able to establish and maintain good working relationships with client, clients family and administrative staff
    • Adequate computer skills to document electronically on a tablet
    • Ability to work independently and as part of a broader care team

    Qualifications:

    • Current Georgia LPN license
    • Current CPR certification
    • One (1) year of clinical nursing experience
