- Precepts multiple learners including IPPE, APPE, PGY1 and PGY2 Pharmacy Residents
- Provides medication information to medical residents, fellows and attending physicians as related to the area of practice
- Applies critical thinking to develop safe and effective therapy plans in real-time to implement during patient appointments
- Orders prescriptions and labs as outlined in the Collaborative Drug Therapy Management (CDTM) Agreement
- B.S. degree in Pharmacy or Doctor of Pharmacy Degree
- Job Knowledge/Experience:
- Current Connecticut Pharmacist License
- Board Certification preferred, would be required to obtain BCACP credentialing within one year of eligibility to participate in collaborative practice agreements in both Primary Care and Anticoagulation Service areas.
