    The Center for Alcohol & Drug Treatment is looking for a part-time (0.5) Medication Navigator to assist our Pharmacist as a part of the Victory Fund Grant. This position guarantees 2-years of employment as a part of the Victory Grant, with opportunity for continued employment based on grant renewal. CADT is looking for an individual with an interest in working with individuals struggling with Substance Abuse and can assist in the continuum of care as directed by the Pharmacist. We are looking for the Medication Navigator to start the first week in July, 2024.

    Summary
    The Medication Navigator will assist the Pharmacist within the residential inpatient treatment and office-based opioid treatment (OBOT) programs. This will be a part of the expanding clinical pharmacy services and education for patients. The Medication Navigator will ensure of the continuum of both medication & human services care. Will assist with Registered Nurse (RN) & Clinical Assistant (CA) needs within residential program related to patient medication.

    Responsibilities:

    • Will help patients with continuum of related care including;
      • Setting appointments with internal or external providers (e.g. medical and mental health appts, dental appts, optometry appts.) and facilitating transportation needs to appts
      • Acting as liaison on behalf of clients for obtaining meds, understanding healthcare systems
      • Advocate for patients' rights
    • Pick-up medication on behalf of patients; following proper chain of custody procedures.
    • Supporting RNs and CAs in medication organization.
      • Ensuring proper organization med rooms & cabinets.
      • Controlled substances are properly stored and organized.
    • Maintain records within Electronic Health Record (EHR) regarding appointments, medications administered, etc.
    • Be collaborative between providers, both internal and external, to ensure patient needs are being met and that continuum of care continues.
    • Respond to calls or emails regarding services provided.
    Required Qualifications:
    • Must have a high school diploma or equivalent.
    • Must have two years of experience within;
      • Medical Social Work
      • Care Coordination
      • Medication Assistant
      • Electronic Health Records
    • Must have knowledge or completed course work in Medical Terminology.
    • Must be able to travel to different program locations as necessary.
    • Detail-oriented, good oral and written communication skills, collaborative, team-focused, flexible, adaptable, and dependable.
    • Commitment to person-centered care for individuals with substance use and/or mental health issues.
    Preferred Qualifications:
    • Experience as a Pharmacy Technician, Certified Medical Assistant, or Certified Nursing Assistant.
    The Center for Alcohol & Drug Treatment completes background checks in compliance with Minnesota Department of Human Services. We must conduct a background for employment within the Center. We do work with applicants on submitting paperwork for reconsiderations and variances if their background requires it.
