    The Referral Coordinator

    provides medical referral management for FSC patients, supporting our mission to provide patient-oriented care coordination.

    This role works closely with Medical Providers and Behavioral Health staff to ensure a full range of health, medical and social services are available to those patients needing specialized or resource-intensive services.

    By arranging medical referral appointments with outside community partners and following up with our in-clinic staff, this highly rewarding role provides direct assistance to those who need it most

    This role is perfect for people who:

    Are proactive, driven, and feed off service interactions with clientele.


    Understand and handle the emotional aspects of communications through the utilization of de-escalation, establishing and maintaining boundaries, problem-solving, and resilience.

    Execute relentlessly through periods of high flow and can get into a groove.

    Thrive in team environments where others can lean on you, and you can lean on them.

    Are comfortable sitting at a workstation and communicating often via phone calls.

    Track their process and make careful notes to keep information organized.

    Speak Spanish (preferred, but this is not essential.)

    Ideally, have experience in areas of social services, psychology, case management, or direct patient care with underserved/indigent populations.

    FSC is here to provide services to vulnerable individuals and families, reducing barriers caused by social determinants of health. Wasatch Homeless Health Care, Inc.

    (doing business as Fourth Street Clinic) began in 1988 as a triage clinic with one part-time nurse, relying on hospitals for patient treatment.

    Today, FSC operates with a staff of nearly 100 caregivers and an eight-figure annual budget.

    Last year, we provided comprehensive health care to more than 5,000 unsheltered men, women, and children through more than 20,000 visits in-person on-site, virtually, or through our Mobile Outreach van.
