- Assists the Resident Care Manager in the organization and efficient functioning of the nursing unit.
- Organizes and maintains medical records according to established standards:
- Completes admission process, maintains consistent order of medical records, updates medical records, completed discharge process
- Manages the data entry of computerized medical record including daily and periodic, updating of Care Plans, MMQ and other medical record components.
- Ensures that vital signs, weights, intake and output are documented in medical records.
- Coordinates the communication of information on resident admission, transfer, and discharge to appropriate disciplines and departments.
- Assists with scheduling of appointments and consultant visits, including completion of referral information.
- Coordinates the Multidisciplinary Team Conferences including dates of conferences, MDS, Trigger/Guideline Reports, RAP Summaries, and Care Plans.
- Ensures that resident identification is accurate and legible.
- Organizes and maintains inventory of resident care supplies and completes billing information for Business Office.
- Coordinates laboratory requests and follow-up.
- Makes environmental rounds to ensure the maintenance and safety of equipment, and the proper identification of resident personal property.
- Acts as the unit receptionist for telephone and direct communication with residents, families, nursing staff, and other disciplines.
- Participates in Quality Improvement activities that promote the effective coordination of communications on behalf of resident care.
- Promotes cooperation among nursing staff and other disciplines.
- Assists with processing orders for resident clothing.
- Assumes additional responsibilities as delegated by the Resident Care Managers.
- Maintain the Daily Census Report and sup to date rosters
- Answer telephone, page calls; deliver messages to residents, etc., as necessary.
- Maintain a current listing of emergency phone numbers for your assigned unit.
- Record appropriate resident identification data on designated medical records, wristbands, ID cards, etc., as required.
- Transcribe physicians' orders to care plans, medication cards, treatment plans, etc., as required.
- Report to the Resident Care Manager discrepancies found in transcribing physicians' orders, diet orders/changes, charting, etc.
- Chart nurses' notes in an informative and descriptive manner which reflects the care provided as well as the resident's response to the care.
- Forward new diet orders and/or diet changes to the Nutritional Services Department
- Keep personnel informed of residents' appointments.
- Assist in arranging for and making resident appointments for diagnostic and therapeutic services.
- Assist in arranging for transportation, packing residents' belongings, escorting them to discharge/transfer area, loading, etc., as necessary.
- Complete necessary medical and administrative records upon the resident's admission, transfer, and/or discharge.
- Direct visitors to resident rooms, office areas, etc., as necessary.
- Attend various committee meetings of the facility
- Assist in the orientation of newly assigned personnel as directed.
- Assist the Nurse Supervisor/Charge Nurse in developing work assignments, schedules, etc., as required.
- Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
- Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.
- Greet newly admitted residents upon admission. Escort to room as necessary.
- Maintain the confidentiality of all resident care information.
- Work in a collaborative effort with fellow employees within the department and with employees in other departments
- Consistently show respect for every person and every position at Sherrill House.
- Consistently communicate important information to others in a proper, timely and professional manner.
- Show respect for fellow employees by following the schedule, reporting on time for work assignments, and assisting with coverage of needed hours.
