    Positions in this function are responsible for checking in, scheduling and staffing patient visits, verifying nursing visits, and generating agency visit authorizations for nursing visits.

    The primary role of the

    Patient Care Coordinator


    is to support the nursing team with scheduling patient visits for home-based and ambulatory infusion suites, where applicable, as well as sending and tracking required plan of treatment nursing orders and supplemental nursing orders.

    The

    Patient Care Coordinator

    also performs other duties as needed based on experience level, including but not limited to assisting with nurse billing tasks

    Schedule:


    Monday - Friday, 8 hr shift anytime between 8:30am - 6:30pm EST w/rotational on-call Saturday shifts once per monthYou'll enjoy the flexibility to telecommute* from anywhere within the U.S.

    Primary Responsibilities:


    Responsible for the coordination and scheduling of patient visits in the home and/or AIS, as applicableAssures timely consistent communication with company personnel and outside providers, in particular with patients, caregivers, and other agenciesTracks/documents and faxes nursing orders (POT/485's) and supplemental physician orders for timely return with appropriate signaturesAssists with triaging of patient phone calls for nursingAssists with the management of FSL scheduling for Optum Infusion NursingAssists with staffing patient visits to subcontracted home health agencies, including sending/retrieving all required documentation for patient managementVerifies appropriate nursing visit authorization is obtained for agency nurse visitsMonitors patient hospitalizations and/or events for completion of the required Performance Improvement Program (PIP) event reports by the reporterGenerates reports through CPR+, CareLink 2.

    0, Nursing Dashboards and Click as requestedAttends/assists in Branch/Regional nurse meetingsAssists with ordering and restocking of nursing supplies as neededVerifies patient demographics during the visit scheduling process at start of care (SOC)Complies with Company policy regarding the timely completion of required training programs (Learn Source and other programs as assigned); completion of common review-self evaluations; and entry of daily time in Global Self-Service (GSS) system

    Required Qualifications:
    High School Diploma/GED or higher1+ years of experience with Microsoft office applications1+ years of previous medical call center experience or medical experienceAbility to work EST Time Zone hours with 8 hr shift Monday - Friday anytime between 8:30am - 6:30pm EST w/rotational on-call Saturday shifts once per month

    Soft Skills:

    The hourly range for California, Colorado, Nevada, Connecticut, New York, New Jersey, Rhode Island or Washington residents is $16.54 to $32.55 per hour.

    Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
