Hanford Post Acute is seeking an Occupational Therapist to join their rehabilitation team full-time.
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Deliver occupational therapy services in accordance with physician orders and care plans
- Document evaluations, daily treatment notes, and progress reports accurately and timely
- Educate residents, caregivers, and staff on therapy techniques and post-discharge care
