This position supports patients and families through clinical assessment, crisis intervention, supportive counseling, case consultation with the health care team and collaboration with other professionals in the community.Responsibilities
- Develop an effective working relationship with patients and families to collaborate, advocate, and problem solve.
- Conduct timely psychosocial assessments.
- Utilize clinical judgment in determining assistance needed by patients and families.
