- Contribute to developing patient care plans.
- Conduct patient assessments, evaluations, and liaise with physicians.
- Implement the Care Plan and maintain accurate electronic records.
- Educate team members, patients, and families. Attend team meetings to coordinate care, exchange information, and problem-solve.
- Adhere to agency policies, Federal and State regulations, and Health Care accreditation standards.
- Coordinate patient care plans, ensuring they are up to date and reviewed regularly.
- Evaluate patient care needs and complete required paperwork accurately and promptly.
- Licensed as an ST / SLP in good standing with the relevant Board.
- Proficient in occupational therapy principles, concepts, and methodology.
- Strong team player with excellent communication skills.
- Ability to build rapport with patients and families.
- Effective problem-solving and decision-making skills.
- Preferably, at least one year of occupational therapy experience in a multidisciplinary setting.
