- Coordinate case management of all patient services.
- Ensure proper documentation for patient care and staff performance.
- Supervise assigned Care staff.
- Provide direct patient care including case management, Oasis care planning, and optimizing staffing resources.
- Active RN licensure in North Carolina.
- Minimum of three years of diverse clinical nursing experience.
- Proficiency in hospice/homecare regulations.
- Strong organizational and critical thinking skills.
- Excellent communication abilities, both oral and written.
- Basic computer skills.
- CPR certification.
