- Serves as a resource to assist providers and their staff in interpretation and application of hospice assessment tools and hospice best practice protocols.
- Offers consultation to providers in person, by telephone, or email.
- Seeks opportunities to assist provider staff in identifying patients appropriate for hospice care.
- Provides care-based education and mentoring for providers and staff
- Identifies opportunities for new and improved hospice involvement and integration into providers' environment as it relates to both staff and patient.
- Demonstrates effective leadership skills.
- Provides an active HRRV presence, where HRRV clients are located along the healthcare continuum.
- Identifies and communicate changes in the provider's environment and the potential impact to hospice services and referrals
- Links healthcare providers with hospice care resources.
- Informs clinical managers of customer service issues in facilities and is a link to provider staff issue resolution.
- Facilitates hospital referrals and admissions to include coordination of discharge from hospital to hospice care.

Qualifications
Qualifications - Licensure as a Registered Nurse
- Five or more year experience in nurse leadership
- Knowledge of hospice services and business practices
Certifications, Licenses, Registrations Required - Current state RN licensure in North Dakota and Minnesota
- Current drivers license and auto insurance
