Summary:

This Position will be responsible for the following:

Oversees chart completion,tracking incomplete and delinquent medical records.
Will serve clinicians with notices ofdelinquent records and will coordinate letters of suspension andreinstatement with the Medical Staff Office.
Analyzes records for completeness andelectronically assigns deficiency tags to clinicians in the EMR.
Will have basic understanding of federal,state and organizational regulations/policy that relate to therelease of medical record information.
Mustmaintain strict confidentiality in all matters pertaining topatients.
Ensures all records for the day'sdischarges or visit for day being analyzed are accounted for andfollows-up with the respective departments if not received.
Registers newborn babies into appropriatereporting systems and submits data to thestate.
Strives for 98% quality andcompleteness to ensure regulatory and organizational requirementsare met.

Responsibilities:

Monitors delinquent records and sends writtennotices of potential suspension to clinicians with delinquentrecords as well as subsequent notices of suspension andreinstatement. These notices are also copied to the Medical Staffoffice for the purpose of credentialing.
Analyzes medical records for deficiencies, flagging deficiencies inthe EMR for clinician completion.
Each Analystmust meet a minimum standard for a satisfactory job performance;minimum compliance will be measured at 98%.
Assists members of the Medical Staff with anyquestions about Chart Completion in HPF.
Assists with patient and provider requests for medical recordsensuring records are released according toHIPAA.
Registers newborn babies intoappropriate reporting systems and submits data to the state,reports and state issues to the HIM Manager.
Conducts quality review to ensure all documents have been scanned,appropriately labeled and accounted for and are visually of highquality.
Answers phones in a polite,respectful and helpful manner. Responds to medical record requestsand questions in a polite, respectful and helpfulmanner.
Analyzes admission and surgical logsto ensure all ops and procedures are dictated timely, servingnotice to clinicians whose reports areincomplete.
Follows up with respectivedepartment for any missing records.
Preparesdaily productivity and provides to manager on a weeklybasis.
Ability to train staff on dailyfunctions.
Strong organizational skills andability to multi-task.
Maintainsconfidentiality and discretion regarding all work matters and workscooperatively with all team members.
Performsall duties in a manner that protects the confidentiality ofpatients and does not solicit or disclose any confidentialinformation unless it is necessary in the performance of assignedjob duties.
Demonstrates competence to performassigned responsibilities in a manner that meets thepopulation-specific and developmental needs of the members servedby the department.
Takes personalresponsibility to ensure compliance with all policies, proceduresand standards as promulgated by state and federal agencies, thehospital, and other regulatory entities.
Demonstrates strong customer service skills with medical staff,patients, and other departments.
Demonstratesexcellent phone etiquette.
Performs otherduties as assigned.

Requirements:

High School Diploma or equivalent years ofexperience required.
3 to 5 years ofexperience preferred.
