Physical Therapist – O/P Rehab Services

Full-time position: Will be a four 10-hr days with two days per week working until 7 pm at Algonquin Rehab Dept.

Prior experience with EPIC EMR is highly desirable but not required; Candidate must be enthusiastic, flexible, resilient, self-starter, collaborative and take pride in providing patient-centered care.

**OFFERING A $5000 SIGN-ON BONUS PAYABLE IN TWO INSTALLMENTS – 50% UPON HIRE; Bachelor's degree in Physical Therapy or above from an accredited school.
May be exposed to hazardous materials and life~threatening diseases.
May be required to work evenings, weekends and holidays.
(Full time/part time staff coming on board at Advocate Sherman have the minimum requirement of working one weekend shift per month and one holiday/year in the inpatient setting.
Associates hired specifically for the outpatient setting must be available to work until 7:00 p.m. two evenings per week in adjunct to the inpatient rehab requirement aforementioned).
Ability to lift up to 35 pounds without assistance.

For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
