Rural Health Transformation Program Manager, Remote Patient Monitoring - Glen Allen - Virginia Hospital & Healthcare Association (VHHA)

    The Virginia Hospital and Healthcare Association is seeking a Rural Health Transformation Program Manager, Remote Patient Monitoring to lead a statewide initiative expanding access to innovative, continuous care technologies for rural communities across Virginia.

    This role within VHHA Foundation manages a multi-partner pilot program funded through Virginia's Rural Health Transformation Program, bringing together hospitals, health systems, vendors, and state agencies to improve care quality, coordination, and long-term patient outcomes.

    Key Responsibilities

    • Serve as project lead for the Rural Remote Patient Monitoring Initiative and primary point of contact for partners and stakeholders
    • Manage vendor selection, contracts, and implementation of continuous care technologies
    • Support hospitals and pilot participants with implementation, technical assistance, and workflow integration
    • Track program performance, outcomes, and data in collaboration with analytics and grant operations teams
    • Ensure compliance with grant objectives, reporting requirements, and funding timelines
    • Explore alternative payment models and reimbursement strategies to support long-term sustainability
    • Build strong relationships with health systems, vendors, clinics, and state agencies
    • Contribute to policy and legislative discussions related to rural health transformation
    • Assist hospitals in identifying and pursuing additional funding opportunities

    Qualifications

    Required

    • Bachelor's degree
    • 5+ years of experience working with hospitals, healthcare providers, nonprofits, or community-based organizations
    • 3+ years leading complex initiatives involving multiple stakeholders, knowledge about clinical practice, and familiarity with efforts to improve community health and relationships
    • Strong project management, data analysis, and communication skills

    Preferred

    • Master's degree in a health or policy-related field
    • Experience with grant writing and securing funding for healthcare or population health initiatives

    Key Skills & Competencies

    • Ability to lead complex, multi-stakeholder healthcare initiatives, coordinating timelines, budgets, and implementation.
    • Understanding of hospital operations, clinical workflows, rural health challenges, and strategies to improve patient outcomes.
    • Ability to monitor, analyze, and interpret performance data to drive quality improvement and inform decisions.
    • Ability to build partnerships with hospitals, vendors, state agencies, and community organizations to ensure successful program adoption and sustainability.

    Work Environment & Travel

    • Full-time, Monday–Friday (8:30 a.m.–5:00 p.m.)
    • Occasional overtime and weekend work as needed
    • Frequent travel within Virginia; occasional U.S. travel
    • Office-based with occasional telework by mutual agreement
