    In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care.

    ResponsibilitiesCollaborate with physicians, patients, families, and healthcare staffReview Care Plan to ensure it is being followed if not, update/ train accordinglyDevelop individualized care plansAccompany clients to Doctor / Hospital visitsCreate goals and monitor progress toward goalsRecruit and train staffCreate schedulesManage 30-40 clientsQualificationsPrevious experience as a Care Coordinator or in a similar position is preferredCertification as a medical assistant is preferredStrong problem-solving and organizational skillsAbility to manage multiple projects or tasks and prioritize appropriatelyAbility to work in fast-paced situations and make sound decisions quicklyExcellent interpersonal skills and high level of compassionStrong verbal and written communication skillsBenefits That Will Make You Go 'Wow':401(k) with matching because we care about your future.
