    Nurse Practitioner – PRN/Part Time

    About the Position:

    During a visit lasting up to one hour, Matrix providers assess a member's current health, medical history, medication adherence, social environment, and other risks. This offers valuable insight into a member's overall health and well-being that can be challenging to capture during regular office visits. The Matrix Comprehensive Health Assessment enhances care quality and allows us to potentially address multiple care gaps in a single visit.

    Responsibilities

    Matrix Provider – What to Expect:

    • Perform Adult/Geriatric Assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation following Matrix guidelines and protocols
    • Collaborate with physicians, case managers, social workers, family members, key caregivers, and relevant ancillary medical staff as needed
    • Work with Primary Care Physician (PCP) on patient education and provide follow-up care
    • Deliver services in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits

    Nurse Practitioner - PRN/Part Time - Home Risk Assessments

    Type: PRN/Part Time - Flexible-Nurse Practitioner

    PAY: $90.00 or $95.00 (county-based) per completed Assessment

    Location: Home, SNF, Telehealth, and other community environments in Warren, Ohio

    Hours: Days, Evening, Nights, weekend, Flexible Scheduling

    Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits except for medical coverage

    Mileage Reimbursement plan


    Qualifications

    Must Haves:

    • Master's Degree required OR equivalent experience and completion of NP licensure
    • Current RN and NP licensure in the state of practice with prescription authority or the ability to obtain it
    • Board certified by the AANP, ANCC, or the AACCN in a Matrix-approved specialty
    • Hold a current BLS, ACLS, or CPR certification
    • Minimum of 1 year experience as a Nurse Practitioner
    • Strong computer skills and familiarity with health/medical record software
    • Excellent verbal and written communication skills with patients, clients, and colleagues
    • Ability to adapt and be flexible in dynamic environments

    Travel Requirements:

    • Able to travel, possess a valid state driver's license, and capable of driving with adequate automobile insurance
    • Travel is essential to reach patients in their residences
