    Description

    HCBS Care Coordinator Social Worker

    This role requires a social worker to provide services on cases involving complex social problems and treatment plans.

    Key Responsibilities:

    • Connect with new contacts and referrals according to health plan guidelines.
    • Maintain communication with members, family members, county staff, community providers for access to services and care coordination.

    Social Work Activities



    • In-person contact per health plan guidelines and ongoing care coordination as appropriate.

    • Maintains documentation in SSIS and Appxtender systems.

    • An annual HRA assessment is completed annually according to heath plan requirements

      → Performs outreach contacts to provide member education, promote use of preventative healthcare measures.


      Maintaining the highest level of customer service by providing the best possible assistance while maintaining confidentiality.
