As a Hospice Liaison primarily for the DeKalb/Sycamore area but will also provide service in the Rockford and surrounding areas, your day blends purpose with strategy.
You will meet with physicians discharge planners and community partners using your hospice knowledge and RN perspective to educate them on services eligibility.
You'll collaborate closely with leadership on marketing initiatives adapt outreach strategies identify new opportunities to serve patients sooner. As an RN your clinical credibility strengthens every conversation.- Serve as knowledgeable liaison between providers hospice services ,
