    Overview:

    The Director of Care Management is a masters level clinician (Nurse Practioner), primarily responsible for the organizations initiatives in improving and sustaining the quality of care provided to Always Compassionate Health's patients. The person in the role will oversee the care management team, the development and implementation of care management supports, and will spearhead the implementation and enforcement of ALL quality improvement initiatives of the organization.

    ***The position is based on site at our Melville/Long Island headquarters, Monday-Friday. Candidates must currently reside in/near Long Island and be comfortable with a daily commute. Occasional local travel to other office locations, as well as home visits will be required.

    Duties and Responsibilities:

    • Classifies active patients roster into their primary and secondary diagnosis and their specific stage of disease progression.
    • Establishes the level of care management support (supportive or intensive) for all patients depending on disease progression, diagnosis and treatment compliance.
    • Classify patients diagnosis and co-morbidities as acute, chronic, and complex health conditions. Based on patients classification of health condition, establish a case management plan for case managers to oversee and manage with the goal of preventing hospitalization, progression of disease and preventing relapse for post hospital discharged patients.
    • Manage patients through an acute medical situation or an exacerbation of a chronic condition, by coordinating with the patients primary care physician to discuss and follow through testing, diagnostics, and necessary interventions.
    • Conduct advanced illness and advanced care planning conversations.
    • Provide patients and caregivers with counseling and education.
    • Work with all care managers in establishing the respective interdisciplinary team of each patient.
    • Routinely review all SOC and bi-annual recertification nursing assessments to ensure accurate assignments and revisions/updates of care management supports for all patients.
    • Establish routine IDT meetings with patients and/or care oversight based on the patients existing and emerging health needs.
    • Collaborate with RN supervisors in all service locations in regard to effective and/or problematic delivery of care management supports.
    • Establish standard operating procedures within the care management unit with the goal of delivering quality care to all patients and immediate crisis interventions when care issues arise.
    • Performs other functions deemed necessary by the DPS.

    Qualifications/Requirements:

    • Candidates must have an advanced degree and training in nursing, specializing in one or more areas in gerontology, community health, pediatric and adolescent care, psychiatric care. ***Nurse Practioner (NP) credentials required.
    • Supervisory/team management experience required
    • Experience establishing and executing quality assurance or performance improvement initiatives
    • Minimum of 2 years work experience in home/community-based care
