    Patients will be referred to pharmacist for medication therapy management. Pharmacists, through telephonic communication, will perform medication reconciliation, assess for any medication-related problems, and provide recommendations to referring physician. Documentation will be completed in patient's electronic medical record (EMR).

    A clinical pharmacy background with an understanding of retail pharmacy operations and strong communication skills is optimal to succeed in this role.

    This role is primarily remote, with expectations to be on-site in West Reading, Pennsylvania for training, quarterly meetings, and from time to time as business needs may require.

    Full time, Monday through Friday, and typically no weekends or holidays required.

    This position is categorized as compliance critical and, as such, requires and is assigned specific responsibility for ensuring continual and proper compliance with all division, corporate and regulatory requirements.

    Cencora manages and staffs three community pharmacies in the West Reading area: the Reading Hospital Ambulatory Pharmacy inside of the 700 bed Reading Hospital, 7th Avenue Pharmacy servicing the Doctors Office Building located on campus, and Wyomissing Pharmacy that shares an office building with the Reading Hospital Endocrinology and Diabetes Center.

    We currently employ pharmacists in both clinical and ambulatory settings with a support staff of technicians in various roles.

    Cencora employs approximately 70 associates dedicated to supporting Reading Hospital / Tower Health, delivering a full spectrum of pharmaceutical care including:

    ~ employee prescriptions fulfillment
    ~ Dual Accredited Specialty Pharmacy with Distinction in Oncology (ACHC)
    ~Complete thorough medication reconciliation.
    Anticipate and address medication questions, such as formulary product selection, product dosing, ADR's, and other considerations.

    Act as financial advocate for patient when needed, leverage charitable foundations, state programs, and/or manufacturer programs to ensure medication affordability.

    Demonstrate ability to navigate various platforms and document appropriately in EMR.
    Cross-train to ambulatory pharmacy for purposes of remote order entry and verification.

    PGY1 residency or previous clinical experience completing medication therapy management activities and providing recommendations to providers is highly desirable.

    Must be proficient in Microsoft Office, including Word, Excel, and PowerPoint.
    Experience in Epic or similar EMR platforms.
    Experience with retail pharmacy prescription processing and claims adjudication.

    This role involves working with broad array of complex disease states and medical conditions; candidates should be comfortable working with clinical data, lab values, and reading/writing notes in patient medical records.


    Normally requires Bachelors of Science Degree or Pharm D degree from an accredited School of Pharmacy a Degree in Pharmacy from an accredited College of Pharmacy; current, unrestricted pharmacist license through the appropriate state board of pharmacy.

    Also requires broad training and/or experience in the fields of business administration, sales, marketing, information technology and retail/ambulatory pharmacy. Normally requires zero (1) to three (3) years of directly related and progressively responsible retail/ambulatory care pharmacy experience.

    Ability to communicate effectively both orally and in writing
    Familiarity with pharmaceutical products, services and drug interactions
    Knowledge of Microsoft Word, Excel, PowerPoint and Outlook
