Team members collaborate with patients' physicians to deliver personalized care either at their homes or our inpatient hospice facility in Durham, NC.
- Clear communication with patients, families, healthcare team, and others.
- Ability to educate patients and families based on the care plan.
- Record-keeping and documentation of interventions.
- Proficiency in using computers and learning new software.
- Graduation from an accredited Practical Nurse program.
- Preferably 1+ years of experience in a Home Health Setting or high acuity, including wound vac experience.
- Active State of North Carolina LPN licensure or Multi-State Compact License.
- BLS Certification (Training provided during orientation if expired).

About the Role:
Shift: 4-10 hour days: Wednesday-Saturday
As a Home Health Licensed Practical Nurse with Duke University Health System, your top priority is delivering excellent service while focusing on the holistic well-being of each patient in a patient-centered environment.
You will be responsible for providing direct nursing care to assigned patients under the guidance of a registered nurse or physician in the patient care area.
You will deliver nursing services to patients and families following the LPN scope as per the North Carolina Board of Nursing guidelines.
