    SCHEDULE AND COMPENSATION:
    On-site position, Monday-Friday. 40 hours per week, some flexibility in hours required as needed to support the needs of the department.
    Salary range $49,160-$56,000/year (Exempt). Salary based on skills and experience
    Up to 6% annual bonus potential.

    ABOUT THE ROLE:

    Under the direction of the Associate Director of Case Management and in accordance with state agency policies and procedures, the Case Manager/QIDP will focus on three key pillars: to advocate, communicate, and coordinate with and for the individuals served at Helping Hand.


    A TYPICAL DAY FOR YOU AT HH:


    Promote the individual's personal development and quality of life in a manner that is determined by the individual and his/her support team in an environment conducive to the qualities of freedom, informed personal choice, and participation.

    Promoting least restrictive environments, optimal community integration, and personal safety are of paramount importance.
    Provide individualized and person-centered services to the individuals served.

    Act as the liaison to coordinate services for individuals served across internal departments as well as external environments which include, but are not limited to; families, ISC Agencies, provider agencies, DHS, Medicaid/Medicare, medical providers, schools, hospitals, rehabilitation facilities, therapists, etc.

    Effectively communicate and coordinate with the individual's Interdisciplinary Team (IDT) and external entities to ensure continuity of care.

    Provide opportunities to increase service satisfaction by responding to the needs of individuals served in a timely and effective manner.

    Ensure that the individual's rights are promoted and protected across all environments, and advocate for the individuals served.
    Ensure that the individual's health is monitored and safety precautions are put in place for continuity of care.
    Create an open and safe environment for the individuals served in order to promote communication.
    Coordinate and facilitate internal and external meetings with and for the individual served.

    Coordinate, monitor, and advocate with and for the individual being served in all areas related to the planning, development, and evaluation processes of person-centered planning and implementation strategies.

    Implement desired outcomes of individuals served to help fulfil their hopes, dreams, and goals for their future by creating clear instructional methods for the IDT to follow and implement.

    Ensure that documentation as part of the person-centered planning and implementation strategy processes are completed on time and in accordance with DHS regulations and Helping Hand's policies and procedures.

    These documents and duties include, but are not limited to; implementation strategy, monthly QIDP notes, ICAPs, SLOFs, medical appointments, home visits, SAMAs & PSRs (HRST), risk assessments, consent packets, self-directed assistance, financial assistance, etc.

    Based upon the services being delivered to the individual, assist the individual in accessing and maintaining funding (SSI, SSDI, Medicaid/Medicare, LINK/SNAP benefits), transportation services, employment, medical services, community integration, and home-based waiver services.

    Monitor services provided to ensure their importance, efficacy, and quality.

    Act as a support to the Case Management department and agency when needed by assisting with in-house clinics, assisting with medical appointments, relaying any important information relating to statewide changes, changes with DHS regulations, change with any external agencies that Helping Hand regularly works with, and/or any changes that effect the way services are provided, and promoting a positive work environment.

    Create, coordinate, and develop non-traditional individualized programs when needed.

    Regularly maintain and update the files of individuals served in accordance with Helping Hand's File Review standards as well as DHS standards is of paramount importance in terms of being reviewed by external entities such as, but not limited to: DHS, OIG, ISCs, CARF, etc.

    Act as the self-directed assistant for clients who utilize Helping Hand services, including updating billing information, create service agreements, create service authorizations, complete monthly temporary increases when applicable, obtain signatures from employers and send said service authorizations to the appropriate agency (ACESS)
    Other duties as assigned by supervisor.
    Qualifications


    LAND THIS OPPORTUNITY BY HAVING:
    Bachelor's Degree in Social Work or a related field.
    At least one year of experience working with individuals with intellectual disabilities.
    QIDP Certification (preferred).
    Exceptional interpersonal and communication skills, both verbal and written.
    Exceptional problem-solving and time management skills.
    Basic computer skills, proficient in Microsoft Word and Excel.
    Valid driver's license with an acceptable driving record and ongoing personal motor vehicle insurance.
    Ability to pass PACE certification requirements (preferred).

    Ability to fulfill all necessary training requirements of the agency, including but not limited to crisis management (crisis management training includes de-escalation strategies and physical management techniques).

    WORK ENVIRONMENT:
    The person in this position needs to occasionally move about to assist clients/students, access file cabinets, office machinery, etc.
    The ability to observe details at a close range (within a few feet of the observer).
    Consistently position self to maintain files in file cabinets or assist clients/students.
    The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
