    Endeavor Health Services is seeking a compassionate and dedicated, full-time Targeted Case Manager/Psychosocial Rehabilitation (TCM/PSR) Specialist to join our dynamic team.

    This vital role enables you to make a significant impact on the lives of individuals with mental health challenges by providing comprehensive case management and psychosocial support. Your expertise will help clients navigate their paths to recovery, fostering hope and promoting independence through personalized care plans and therapeutic interventions.

    Responsibilities:

    • Provide targeted interventions to support the wellness and recovery of clients with complex or chronic behavioral health issues

    • Refer clients to community services and assist in overcoming behavioral health issues interfering in daily functioning

    • Advocate for family support and required services through management and monitoring processes

    • Work effectively with child, adolescent, and adult clients

    Qualifications:

    • Bachelor's degree in a Human Services field is required; Master's degree is preferred

    • Experience in a similar position is necessary

    • Electronic health records software experience (Cerner) is a plus

    • Excellent communication skills are essential

    Salary: $20.00 - $22.00 per hour, depending on education and experience.
