Purpose Neighborcare Health's Homeless & Housing Programs is seeking a primary care provider to join our teamWe are passionate in providing high-value, patient centered health engagement and health care to our community members who are experiencing homelessness, to those living in permanent supportive housing, and others in our community.We are driven by the belief that all persons have the right to housing and health care.Through innovative models of care and inter-professional teams, we strive to make our care as accessible as possible for our clients. Read more about our work here: In this position you will: Direct Clinical Work
    • Provide patient centered preventative care for adults and adolescents, as well as diagnoses and treatment of acute and chronic conditionsin collaboration with interdisciplinary team (nurse, behavioral health, patient navigator, social work, diabetes educator).
    • Identify the need for referrals and provide overall care coordination in a team-based setting.
    • Work with individuals of varying ethnicities, socio-economic levels, cultural and sexual orientation.
    Indirect Clinical Work
    • Assure accurate and timely completion of all clinical records; completion of paperwork in a team-based approach.
    • Complete necessary paperwork for patient care, such as, prior authorizations and communications with outside specialists; timely response to communications and test results
    Provide feedback to appropriate teams and/or individuals when necessary KEY SKILLS, KNOWLEDGE, & ABILITIES Preferred qualifications for this position include :
    • Preference will be given to providers with at least 1-year of experience in community health, FQHC, family practice
    • Experience in MOUD and poly-substance use disorders
    • Experience with gender affirming care,PrEP, hepatitis C treatment, people experiencing homelessness,treatment of acute and chronic conditions
    • DEA license
    • Contraceptive Implant Certification
    • Bilingual skills

    Required Qualifications
    • Graduation from an accredited School of Medicine or Osteopathy
    • Completion of residency (for MD/DO)
    • Licensed as a Physician in the State of Washington
    • Board certified
    • Current BLS certification
    • DEA License issued in the State of Washington
    Preferred Qualifications
    • Additional training in Addiction Medicine
    • 1+ yearsof clinical practice experience in community health, FHQC, family practice
    • Bilingual skills
    • Experience prescribing buprenorphine or other treatments for opioid use disorders, including past completion of a buprenorphine waiver certification course or specific continuing education to satisfy the requirements for the DEA's Medication Access and Training Expansion (MATE) Act.

Compensation will be determined based on years of relevant experience.
