    Overview

    We are launching a new branch for Palliative Care Advanced Illness Management in Lufkin.

    We are on the lookout for a dedicated Care Coordinator AIM to become a part of our team. This role involves reporting directly to the Director of AIM and overseeing the day-to-day business operations and other activities related to advanced illness management (AIM) and palliative care department.

    Essential Functions of Position

    • Coordinate and manage all operational activities for the AIM and palliative care department.
    • Process referrals, ensuring timely and suitable admission of patients.
    • Maintain communication with patients, families, and referral sources.
    • Organize scheduling for Nurse Practitioners efficiently.
    • Assist in securing appropriate reimbursement and verify benefits.
    • Manage clinical records functions in compliance with regulations.
    • Submit necessary billing information to the Billing Department promptly.
    • Establish and maintain relationships with referral sources.
    • Act as a resource for customers and the community regarding AIM, Palliative Care, and Hospice.
    • Participate in quality assurance and performance improvement activities.
    • Implement marketing initiatives as directed.

    Requirements

    • High school graduate or equivalent. RN or LPN preferred.
    • College graduate is a plus.
    • Minimum of three years of healthcare delivery or related business experience.
    • Experience with Computer applications and Electronic Medical Records.
    • Knowledge of medical terminology and insurance reimbursement process.
    • Effective data entry, problem-solving, human relations, and communication skills.
    • Some travel may be required.
