    Description

    About this position

    The Licensed Practical Nurse is responsible for providing individualized care to customers and their families according to established standards, policies, and procedures. Dependent on clinic, customer populations may include infants, children adolescents, adults and geriatrics. The LPN provides observation, monitoring, and intervention when necessary.

    Occupational Medicine: Responsible for providing individualized care to customers in an employer on-site and off-site environment.

    Unit/Department Specific:
    Clinic hours and available shifts vary by location. Some positions may require weekend rotation.

    Atrium Specialty Clinic

    • Completes pre-visit planning to prepare the care team for the customer's visit

    • May assist with prescription refills under supervision of the provider according to the Clinic Medication Refill Protocol

    • Assists the provider and other members of the customer's care team in communicating about the customer's care plan both during and outside of the customer's clinic visit. Will provide assistance in coordination of transportation, surgery scheduling, and other follow up care as needed.

    • Assists in the coordination of follow-up care within the Atrium Specialty Clinic when a customer has been discharged from our provider's care in the hospital.

    Mental Health Services Center

    • Completes pre-visit planning to prepare the care team for the customer's visit

    • May assist with prescription refills under supervision of the provider according to the Clinic Medication Refill Protocol

    • Assists the provider and other members of the customer's care team in communicating about the customer's care plan both during and outside of the customer's clinic visit. Will provide assistance in coordination of follow-up care as needed

    • Ability to complete DISCUS testing, department will provide training if needed

    Requirements

    Education


    • High School diploma or equivalent (, GED) required.

    • Graduate from an accredited Licensed Practical Nursing or Vocational School required.

    Experience


    • Previous clinic experience preferred

    • Previous EMR experience preferred.

    • Experience in lab and x-ray may be required depending upon clinic needs.

    • Occupational Medicine: Occupational medicine experience preferre
    • Demonstrated leadership, verbal, written and interpersonal communication skills required.

    • Occupational Medicine:

    • Travel using personal vehicle to various local worksites required

    • Oncology/Hematology:

    • Competent in vascular access of implanted ports for lab draws and competent in venipuncture.

    • Knowledge of documentation and notification of panic values, rain rating, and abnormal lab values to provider/PNC nurse.

    • Competent in infection control policies and procedures, including care of MRSA, VRE and contact precaution customers.

    Licensure/Certification


    • Current licensure as LPN or LVN in state of Minnesota - State Requirement

    • Current BLS Certification-NMHC Standard
