    Title:
    Nurse Practitioner - Advanced Practice Provider

    Schedule:
    Full-Time, Monday through Friday, 8:00 AM - 5:00 PM


    Role Description:


    The main focus of a Nurse Practitioner at Oak Street Health is to offer quality primary care to older adults on medicare to ensure they stay healthy, happy, and away from hospitals.

    Nurse Practitioners work independently with patients and collaborate with physicians as needed.

    They deliver ongoing care, establish important patient connections, and build trust to help patients engage in their care plan.


    Nurse Practitioners operate in outpatient clinics as part of a collaborative team including a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers.

    The organization partners with top specialists and hospitals for specialized and acute care.

    Thus, healthcare providers can concentrate on clinic-based activities like:
    primary care, care coordination, and population health.

    Core Responsibilities:
    - Providing exceptional primary care
    - Conducting office visits for routine and acute issues


    - Administering Annual wellness visits and health risk assessments, focusing on thorough documentation

    - Coordinating care with other providers and facilities
    - Leading population health initiatives, ensuring all eligible patients receive scheduled screenings
    - Assisting with phone triage and patient outreach
    - Educating patients on their health conditions and treatments
    - Participating in Oak Street Health's promotional activities
    - Conducting home visits as required
    - Performing other assigned duties

    This role reports to the Center Medical Director and closely collaborates with operational leaders.

    Required Qualifications

    - Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
    - National certification as a Nurse Practitioner through AANP or ANCC
    - Active state Nurse Practitioner license
    - Active DEA license
    - US Work Authorization
    - Demonstrate core values of being "Oaky"

    Preferred Qualifications:
    - Experience in primary care: internal medicine, geriatrics, or family medicine
    - Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
    - Team player with a passion for collaborative work
    - Interest in innovative population health models
    - Bilingual proficiency in relevant areas
