We are inviting *Registered Nurses* to join our growing team in the *Rock Hill / Fort Mill, South Carolina* and surrounding areas by providing *full time Case Management *services to patients in their homes, assisted living and long term care facilities who are enrolled in our *hospice program.

• Provide patients and families with the care and compassion they deserve at this time in their lives.
• Coordinate care with a team of professionals, including medical clinical director, medical social workers and other passionate caregivers.
• Collaborate with Physician for new orders, to advise of needs and/or changes.
• Provide education to patients and families

• Must have a RN license eligible in the state you are applying
• Willingness to be flexible and work some on-call hours as needed on a rotation
• One year of working nursing experience required
• Hospice experience preferred but not required
• Reliable transportation and valid drivers license
