The Certified Community Health Worker is responsible for serving as the initial and main point of contact between the company and the member.
Supports member access to care through face-to-face visits to complete health screening and needs assessments. Certified Community Health Worker Refers member to community resources, processes reports, and distributes collateral materials.Competencies/Duties:
Certified Community Health Worker coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs.
Will provide telephone and/or on onsite Recovery SupportMakes referrals to the Case Management ServicesAssesses member compliance with medical treatment plans via telephone or through on-site visitsCertified Community Health Worker identifies barriers to plan compliance and coordinates resolutions while ensuring alignment with internal company teamProvide assistance accessing various community resources (education, housing, transportation, recovery supports, etc.)Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and providerCertified Community Health Worker coordinates identification of and referral to local, state or federally funded programsParticipates in cross-functional teams on projects, initiatives, and process improvement activitiesMaintains the confidentiality of all sensitive information.
Accountable to understand role and how it affects utilization management benchmarks and quality outcomes RequirementsPosition Requirements/Abilities:Ability to complete 35 community visits per weekStrong interpersonal and communication skillsAbility to communicate effectively with people from a diverse socio-economic levels and backgroundsSuperior organization skillsMust be able to work independently and effectively with othersHigh degree of computer literacy is preferredAbility to handle protected health information and personally identifiable information in accordance with HIPAA requirements.
Must be fully vaccinated against COVID-19Education, Work Experience, Licensure:Candidate must reside within New Albany/Jeffersonville, Indiana areaCertifications/licensures required: Community Health WorkerAbility to obtain a valid Indiana driver's license, proof of valid insurance, and access to reliable transportationHigh School Diploma or equivalent GEDOne-three years of experience in health services or behavioral health field working in a diverse community-based settingWorking knowledge of community-based populations, medical conditions and/or disabilitiesEnvironment/Physical Demands:
Ability to continuously stand or walk.
Ability to bend, climb stairs with the potential for frequent liftingAbility to lift up to 25 pounds, occasionallyThe remote position requires the individual to work from home or established work site.
Must have dedicated space within home to operate fully.This position requires frequent driving which requires the ability to sit for long periods of time. Must provide proof of a valid Driver's License and valid insurance coverage.
This position requires the employee to conduct home visits in members' homes which requires the individuals to obtain and exchange information and assist the member with resolving simple to complex issues related to their accessing their health care benefits.
