- Enroll patients into a comprehensive preventive health program.
- Work closely with clinical staff to create individualized preventive care plans for patients.
- Conduct Initial Patient Visits per Precision Health protocol in a clinic setting and occasionally a patient's home.
- Educate patients on Remote Patient Monitoring (RPM), Precision Health, and other telehealth programs.
- Manage patient data across multiple systems including EHR, patient portals and other compliant data storage systems.
- Monitor patient vital signs, such as pulse rate, temperature, blood pressure, and weight, and record information in patient records, creating customized care plans for them.
- Engage with patients by phone, text, and video to: Remind them to record their vitals; Respond to alerts related to symptoms or physiologic data; Respond to minor technical troubleshooting and provide ongoing chronic care management.
- Review the vitals, communication of results, and patient concerns to Precision Health and Primary Care Physicians as appropriate.
- Assist with device ordering, managing patients, ensuring patient compliance, and generating reports to ensure the practice is reimbursed in a timely manner.
- Maintain call logs for patients.
- Proven experience as a registered nurse
