- Conduct comprehensive resident assessments in accordance with regulatory guidelines and facility policies.
- Coordinate and oversee the completion of the Minimum Data Set (MDS) for each resident, ensuring accuracy and completeness.
- Collaborate with interdisciplinary teams, including nursing staff, physicians, therapists, and social workers, to gather information for resident assessments.
- Analyze assessment data to develop individualized care plans that address the physical, psychosocial, and emotional needs of residents.
- Maintain up-to-date knowledge of regulatory requirements related to MDS assessments and ensure compliance with state and federal regulations.
- Monitor and track changes in resident conditions, updating care plans as needed to reflect changes in health status or treatment plans.
- Serve as a resource for staff regarding MDS coding, documentation requirements, and regulatory compliance.
- Participate in quality improvement initiatives and regulatory surveys, providing documentation and support as needed.
- Provide education and training to staff on MDS assessment processes, documentation requirements, and regulatory updates.
- Maintain confidentiality of resident information and adhere to HIPAA guidelines in all aspects of job performance.
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid nursing license.
- Minimum of 2 years of clinical experience in a long-term care or skilled nursing facility.
- Certification in MDS assessment preferred.
- Strong understanding of nursing care planning processes and regulatory requirements.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with interdisciplinary teams.
- Proficiency in computer skills, including experience with electronic health records (EHR) and MDS software.
- Attention to detail and accuracy in documentation and data entry.
- Ability to prioritize tasks and manage time effectively in a fast-paced environment.
- Commitment to maintaining compliance with state and federal regulations and upholding ethical standards in resident care.
- Continuous commitment to professional development and staying updated on changes in regulatory requirements and best practices in long-term care.
- The MDS Coordinator primarily works in an office setting within a long-term care or skilled nursing facility.
- May require occasional travel for training or professional development opportunities.
- Must be able to work flexible hours to accommodate resident assessment needs and regulatory deadlines.
