- Conduct psychosocial assessments, provide direct casework, and counseling services to patients and families.
- Collaborate with the Interdisciplinary Team to develop and execute comprehensive care plans; Effective communication and collaboration includes attending Interdisciplinary Team meetings, staff meetings, and other requested meetings.
- Take primary responsibility within the Interdisciplinary Team for Advance Directives, Bereavement referrals, APS referrals, funeral planning, volunteer referrals, correct demographics in the EMR, current insurance information, and paperwork related to discharges and level of care changes and placements.
- Maintain accurate clinical documentation according to Hospice standards.
- Adhere to the NASW Code of Ethics and organizational policies, ensuring confidentiality of patient, family, and staff information.
- Communicate with outside agencies, including Region 10 and Social Services to stay updated on community resources and facilitate referrals to these resources as needed.
- Stay current on hospice and grief-related topics/research through agency and community programs.
- Demonstrate knowledge of emotional, social, and spiritual needs in addressing death and grief.
- Orient volunteers and staff; supervise field placement students when applicable.
- Education and Licensure: Master of Social Work (MSW) degree. Current state licensure (LCSW) preferred.
- Experience: Two years of social work experience, with at least one year in a healthcare setting. Clinical experience with individuals, groups, and families.
- Knowledge: Knowledge of death, dying, and bereavement processes. Understanding of social work principles, counseling techniques, hospice philosophy, and group dynamics.
- Skills: Proficiency in Microsoft Word software and EMR systems. Excellent interpersonal and communication skills. Basic computer skills.
- Mindset: Team player, flexibility, and ability to function in stressful situations.
