    About The Job

    Salary: NJC Grade depending on experience: £37,336 to £44,428
    Working Pattern: Full time
    Contract Length: Permanent
    Location: Horsham

    The aim of Adult Care Point 2 - Initial Contact (ACP2) is to meet need at the earliest opportunity in a person's journey. As part of a multi-disciplinary team that supports, prevents, reduces and delays the need for long term services, you will provide specialist advice to social care professionals and social workers within the ACP2 team.

    You will work as part of a strong and dynamic Occupational Therapy service, undertaking strengths-based assessments (as directed by the Care Act 2014) utilising Occupational Therapy skills and knowledge to develop and implement a plan of support, aimed at improving wellbeing and delivering outcomes for people.

    You will be required to undertake a duty function, completing telephone assessments of needs with customers approaching social care for advice, information, equipment and adaptation (e.g. stair lifts and level access showers). You'll be expected to use up to date evidence-based practice to inform your intervention.

    Physically, you will need to be able to sit for prolonged periods of time during the telephone assessments using a head set and or iPhone.

    Experience and Skills

    Key Skills:

    1. Ability to apply a risk enablement approach and use evidence-based practice.
    2. Ability to complete a strengths-based assessment (as directed by the Care Act Ability to develop a person-centred care and support plan with clearly defined outcomes.
    4. Excellent communication and interpersonal skills in order to interact effectively with a range of stakeholders to provide clear advice and guidance and clinical interventions.
    5. Ability to prioritise and organise work and meet agreed performance expectations.
    6. Sound and accurate IT skills to ensure effective record keeping, respecting confidentiality.

    Qualifications and/or experience:


    • Diploma / BSc Occupational Therapy or equivalent Occupational Therapy qualification

    • Registration with Health and Care Professions Council (HCPC)

    • Experience of undertaking clinical functional assessments and of Occupational Therapy intervention planning, implementation and review

    • Experience of working as an Occupational Therapist with people with physical disabilities in the community providing appropriate interventions to enable independence at home

    • Experience of inter-agency and multi-disciplinary work to support collaborative working relationships
