Job summary
Provide pharmaceutical care services to the patients of the Duke University Health System including but not limited to oversight monitoring preparation dispensing and proper documentation of patient medication therapy as well as providing drug information patient information and oversight of information and dispensing systems; ensure compliance with federal state and local laws and regulations as well as proper storage and documentation.
+Responsibilities
- Review monitor patients medication therapy orders written by appropriate caregivers for safety efficacy considering specific patient conditions health status advise medical team on issues possible enhancements interventions document such actions maintain accurate records files in accordance with Federal State local laws regulations regarding drug controls Joint Commission standards oversee maintenance required records systems charge credit medications treatments provide timely accurate reports recommendations related issues affected processes advice healthcare providers staff patients students parents pharmacists nursing staff other departments needed support education pharmacy residents participate multidisciplinary teams evaluate progress develop implement individualized treatment plans improve outcomes enhance quality life patients served area direct responsibility assist others department same conduct research investigate impact delivery quality care examine pharmacoeconomic effects administering medications treatments perform duties incidental work described herein.
