    Bargaining Unit: Non Represented - Manager

    Rate of Pay: $149,032 annually + DOE

    Summary

    The Care Coordinator Nurse Manager is responsible for overseeing the operations for the Clinical Coordination Department. Oversees all administrative issues, concerns, and communications; facilitates problem solving and personnel issues. Responsible for coverage of department in time of need.

    Essential Duties and Responsibilities
    • Develops and ensures compliance with departmental policies and procedures.
    • Assures quality care as defined by clinical outcomes.
    • Acts as a clinical resource
    • Ensures that confidentiality of hospital and patient information policies and guidelines are observed.
    • Develops operational budget recommendations for submission to the CNO
    • Responsible for time-care payroll adjustments, variance logs.
    • Supports patient self-management of disease and behavior modifications interventions.
    • Assures home monitoring and patient knowledge that is consistent with treatment plan.
    • Coordinates continuity of patient care with internal and external healthcare providers.
    • Coordinates continuity of patient care with patients and families following hospital admission, discharge, follow up calls and ER visits.
    • Conducts home visits as indicated to monitor patient progress and assess needs.
    • Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting.
    • Conducts comprehensive, preventive screening for patients and/or assists all support staff in daily patient interactions a needed.
    • Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans..
    • Participates on a team for data collection, health outcomes reporting, and clinical audits.
    • Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
    • Assists in the development of plan of care in collaboration with the health care team.
    • Oversees Health Promotora to assist with patient and family treatment care planning.
    • Demonstrates System Values in performance and behavior.

    Supervisory Responsibilities
    Carries out supervisory responsibility in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, training, assigning, coaching, counseling, and disciplining employees; administering scheduling systems; communicating job expectations; planning, monitoring, appraising, and reviewing job contributions; enforcing policies and procedures.

    Minimum Education/Experience
    Bachelor's Degree Degree from college or vocational school from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty.

    2-3 years relevant experience

    Required Licenses/Certifications
    California Registered Nurse (RN) - Upon hire

    Nevada Registered Nurse (RN) - Within 6 months of hire into job

    BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification). - Within 3 months of hire into job
