Physical Therapist (PT) - PRN Job Opportunity

Little Sisters of the Poor Sacred Heart in Mobile - SNF Location
Premium Compensation Range: $55-60 per hour

The Physical Therapist role involves evaluating and treating residents based on physician orders. This includes using various therapeutic procedures to enhance muscle strength, coordination, pain relief, and overall movement and function improvement. The Therapist works collaboratively within the interdisciplinary team to provide necessary support and information in the field of Physical Therapy.

Responsibilities

1. Analyze resident information to guide the assessment and treatment process effectively.

2. Perform a comprehensive functional assessment of residents and identify their specific needs.

3. Create and execute personalized treatment plans with achievable goals.

4. Ensure adherence to evaluation, treatment, and documentation standards.

5. Train Assistants on treatment procedures and provide supervision during resident interactions.

6. Maintain accurate records of each treatment session.

7. Adjust treatment plans as required throughout the therapy process.

8. Summarize the treatment course concisely in discharge reports.

9. Educate residents on program services and potential risks involved.

10. Collaborate with nursing staff, residents, and families to optimize residents' functional abilities.

11. Instruct residents and caregivers on post-treatment exercises and procedures.

12. Assist with referrals for additional services post-discharge and stay informed about community resources.

13. Participate in rehabilitation team conferences to discuss resident progress and needs.

14. Attend necessary department and facility meetings.

15. Pursue relevant continuing education opportunities to maintain clinical competency.

16. Contribute to quality improvement initiatives.

17. Advocate Heritage Healthcare programs and services through community interactions.

18. Adhere to professional standards and guidelines.

19. Provide feedback on the performance of Assistants to the Rehab Manager.

20. Follow facility policies and participate in quality improvement and safety programs.

21. Perform additional duties as needed to ensure quality resident care.

22. Understand payer reimbursement systems and allocate therapy resources efficiently.

23. Demonstrate knowledge of relevant regulations and guidelines.

24. Uphold the company's commitment to Compassionate Care.

25. Work independently and seek supervision when necessary, fostering teamwork.

26. Maintain expected productivity levels as discussed.

Qualifications

1. Bachelor/Masters of Science degree in Physical Therapy.

2. Valid state license for practice.

3. Preferable experience in a relevant setting.

4. Certification in specialized areas, if applicable.

5. Strong communication and interpersonal skills.

6. Proficiency in physical therapy principles and techniques.

7. Ability to assess resident needs and develop comprehensive care plans.

8. Familiarity with accreditation standards.

9. Capable of performing physical tasks safely and effectively as part of the job requirements.
