    Job DescriptionMinimum 1+ years pharmacy experience required.

    We are seeking a dedicated and experienced Staff Pharmacist to join our team at a reputable long-term care facility in Bethlehem, Pennsylvania. This position involves working the mid shift, providing crucial pharmaceutical services to our residents. The ideal candidate will demonstrate a commitment to patient care, a thorough understanding of geriatric pharmacology, and the ability to work collaboratively within a healthcare team.

    Key Responsibilities:

    • Review, interpret, and dispense medications accurately and efficiently.
    • Conduct medication regimen reviews and provide recommendations for optimizing therapeutic outcomes.
    • Monitor patient profiles for drug interactions, contraindications, and allergies.
    • Collaborate with healthcare providers to develop and implement patient care plans.
    • Provide drug information and education to patients, caregivers, and healthcare professionals.
    • Ensure compliance with all state and federal regulations governing pharmacy practice.
    • Participate in quality improvement initiatives and contribute to the overall safety and efficacy of medication management within the facility.
    • Maintain accurate and complete patient records, ensuring confidentiality and compliance with HIPAA regulations.

    Shift:

    • Monday - Friday: 11:00am - 7:00pm
    • Rotating weekends

    Qualifications:

    • Pharmacy experience.
    • LTC experience a plus.
    • PA Pharmacist license.
