    I) Job Purpose


    The Continuity of Care Specialist (COC Specialist) functions as part of an Interdisciplinary Team (IDT) to provide, psychosocial support, supported employment services, crisis resolution, and housing services to adults who have evidence of psychiatric or co-occurring psychiatric and substance use disorders (COPSD), and who are currently on parole/probation and who need intensive rehabilitative and casework mental health services.

    The COC Specialist provides a method of comprehensive review and evaluation of both mental health and criminal justice history to determine the least restrictive and clinically beneficial treatment disposition.

    This individual is required to provide coordination, advocacy, interaction, and cooperation with other departments both external and internal to MHMRTC.

    The COC Specialist provides assessment, recovery planning, and ongoing treatment when applicable in order to assist individuals with severe and persistent mental illness to gain the skills, resources, and supports necessary to attain their individualized recovery goals.

    Services may be provided in the clinic or in the community.

    Providing services in the community entails travel (often in the COC Specialist's own vehicle) and work throughout Tarrant County, in a variety of neighborhood and home conditions.


    II) Essential Functions
    A) Complete intakes on all TCOOMMI referrals including jail releases, prison releases, Probation and Parole referrals.


    B) Assess TCOOMMI referrals for TCOOMMI services and eligibility including Intensive Case Management (ICM), Transitional Case Management (TCM) and Continuity of Care (COC) services.

    C) Link and refer TCOOMMI Patients to community resources and benefits.

    D) Monitor TCOOMMI Web App daily for new referrals.

    E) Update TCOOMMI Web App with closures, county to county transfers and appointment status.

    F) Collaborates with Program Directors/designee at VOA, ABODE, Core Civic Halfway House Probation and Parole Departments to research, plan, and develop systems to improve the service delivery of the Project R.A.P.P. program.

    G) Comply with TCOOMMI Program Guidelines and Procedures including Intake, Continuity of Care, Intensive Case Management, Transitional Case Management and Halfway House.

    H) Document all contacts and attempts to assist Patient with community resources and TCOOMMI/MHMR Tarrant services in the Electronic Health Record.

    I) Complete documentation of services provided daily by 10 am the next business day.


    i) Standard:
    (a) All documentation must be completed, processed (including corrections), and entered into the agency's data system no later than within the time frame designated by current division policy.

    Utilize Collaborative Documentation when providing services to an individual.

    J) Attend and participate in all Recovery Team Meetings


    i) Standard:
    (a) COC Specialist obtains prior approval from his or her Team Lead or Program Manager for every Recovery Team Meeting not attended.

    K) Maintain credentialing as a QMHP.


    i) Standard:
    (a) No Relias training is 60 days or more overdue at any time.

    (b) COC Specialist always maintains clinical supervision/licensure/requirements.

    L) Maintain timekeeping and travel records daily, ensuring final records are turned in and approved before the close of each pay period or as requested by supervisor.


    i) Standard:
    (a) COC Specialist has no more than two missed punches (clock-in or clock-out) in the timekeeping system in any given pay period.

    (b) COC Specialist notifies his or her supervisor immediately if a missed punch occurs.

    N) Perform other job duties or responsibilities as requested or assigned.


    III) Knowledge of Laws, Policies/Procedures, Skills, and Abilities
    A) Applicable sections of Texas Administrative Code, Texas Department of Criminal Justice TCOOMMI Standards, Requirements, and Procedures, Third Party Payer Requirements, MHMRTC Policies and Procedures, Mental Health Code, Medicaid Procedures, Compliance Plan, MHMRTC Employee Handbook, HIPAA, and applicable Federal standards.

    B) COC Specialist demonstrates effective organizational, time management, and communication skills.

    C) COC Specialist is self-motivated and self-directed demonstrating the ability to work autonomously.

    D) COC Specialist provides excellent customer service both within and outside the agency.

    E) COC Specialist demonstrates the ability to engage individuals in services while maintaining appropriate professional boundaries.

    F) COC Specialist demonstrates the ability to work in a non-judgmental manner with individuals and coworkers whose belief systems may be based on values different from those of the QMHP.

    G) COC Specialist demonstrates the ability to communicate clearly and concisely both orally and in writing.

    H) COC Specialist demonstrates the ability to work as a member of a multidisciplinary treatment team.

    I) COC Specialist demonstrates the ability to accept direction and make constructive use of professional and clinical supervision.


    IV) Internal & External Customer Service


    This position requires extensive internal and external contacts with clients, family members, significant others, inter-agency staff, service providers, and practitioners.

    The COC Specialist accomplishes communication with exceptional written and verbal skills, by answering phone calls and emails within one business day, and by conducting themselves professionally in all interactions.

    Other contacts may include, but are not limited to:

    John Peter Smith Hospital, Family Services, Social Security Administration, Department of Assistive and Rehabilitative Services (Department of Regulatory and Protective Services, other area hospitals, local Police Department(s), area United Way Agencies, local housing authority and/or local Transportation Authority.


    V) Travel
    A) This position requires significant travel within Tarrant County. A company vehicle may be available, or you may be expected to use your personal vehicle.

    Staff member is responsible for maintaining accurate records and submitting them on time in order to receive compensation allowed per division policy.

    Staff member is responsible to pay in full any traffic tickets and fines for traffic violations while working.

    Tickets received by agency due to a staff member's traffic violation while driving a company vehicle are deducted from staff members paycheck.

    Minimum Requirements


    Job Requirements:

    Required Education
    Bachelor's Degree in a Health or Human Services Field.


    Defined Education
    Social, Behavioral or human services include Psychology, social work, nursing, rehabilitation, counseling, sociology, human development, deontology.


    Substitutions


    May substitute a Bachelor's Degree with at least 30 hours of behavioral health coursework which must be documented within the transcript.


    Experiences
    1 year


    Defined Experience
    1-year experience in a Health or Human Services Field.


    Required License(s)
    i) Valid Texas Driver's License

    ii) Proof of Liability Insurance Coverage

    iii) Successful completion of required organizational training

    iv) Qualified Mental Health Professional-Community Specialist (QMHP-CS)
