- Provide oversight of clinical and Nurse Practitioner services in coordination with the Medical Director
- Assist with on-call needs when the Medical Director is unavailable
- Provide oversight on initial and ongoing assessment of the impact of the terminal diagnosis on daily living, and the patient's physical, functional, psycho-social, and environmental needs
- Partner with an interdisciplinary team to implement and manage adequate, individualized plans of care, determining scope, priority, and frequency of services based on acuity and patient/family needs
- And perform additional, related duties as needed

- Have at least 1yearof Nurse Practitioner work experience, preferably in hospice
- Have an active Advanced Practice Registered Nurse (APRN), CNP, or other NP license
- Are able to travel within the service area of Brighton (current driver's license, personal vehicle, and auto insurance)
