Job Summary
The CSP/HRSN Community Health Worker (CHW) provides intensive case management services to individuals who have a Health Needs Based criteria (HNBC) and risk factor as defined by Mass Health for CSP and HRSN Supplemental Housing Services.The CHW coordinates completion of intake assessment treatment plan conducts initial ongoing risk assessments designs crisis prevention relapse prevention harm reduction strategies collaborates closely with primary care providers assures appropriate referrals based on level of care needed optimizes outcomes minimizes risk assists members improving daily living skills helps members access benefits housing healthcare accompanies members essential healthcare appointments assists obtaining benefits housing health care collaborates emergency service programs mobile crisis intervention develops revises utilizes crisis safety plans fosters empowerment recovery wellness forms connections recovery-oriented peer support self-help services manages all transitions through collaboration community provider staff hospital staff ensures safe discharge plan well-coordinated implementation completes service notes case documentation electronic health record accurate timely billing participates routine multidisciplinary team meetings completes assigned courses training workshops best practices assigned supervisor provides pre-tenancy supports tenancy transition tenancy sustaining supports performs outreach in-home assessments remediation needs arranges contractor identified remediation need uses evidence-based promising practices optimize care integration effective communication systems rehabilitation recovery wellness principles housing first approach develops maintains relationships internal external parties agencies providing service persons homelessness at-risk tenancies uses person-centered culturally competent strengths-based approaches help members find get keep housing ensures consistently provided trauma-informed culturally responsive services performs other related duties.
