- Provides oversight and guidance for the Anticoagulation Clinic Service.
- Performs drug regimen / laboratory data review and adjusts anticoagulation therapy, when appropriate, to maximize outcomes for all Anticoagulation Clinic Service patients.
- Provides appropriate patient monitoring to ensure that desired outcomes are met by following Anticoagulation Clinic protocol and Chest Guidelines
- Educates patients and families regarding anticoagulation therapy.
- Maintains organized documentation on recommendations, interventions, follow-up actions and outcomes in Coagclinic and Webcare.
- Communicates effectively with prescribers referring patients to the Anticoagulation Clinic.
- Maintains competency in anticoagulation care. Serves as an active patient advocate regarding anticoagulation therapy Uses a team approach to working with staff and others
- At a minimum, a BS in Pharmacy from an accredited college of pharmacy. PharmD preferred.
- Iowa Pharmacist Licensure, either by examination or by reciprocity.
- Demonstrated competence in providing direct patient care.
- Previous anticoagulation management experience preferred.
- Self-motivated, detail-oriented individual able to work with minimum supervision.
- Good communication skills.
- Leadership skills.
- Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
