    Job Description Cape Abilities is looking for the best RNs and LPNs to join our outstanding nursing team Bring your kindness, compassion, and exceptional care to support individuals in our residential programs across Cape Cod.

    In conjunction with residential management, our next Residential Nurse will provide a comprehensive system of residential services, creating a safe and therapeutic environment that fosters the development of human dignity and focuses on personal well-being.

    GENERAL SUMMARY

    In conjunction with residential management, provides a comprehensive system of residential services creating a safe and therapeutic environment that fosters the development of human dignity and focuses on personal wellbeing.

    ESSENTIAL JOB FUNCTIONS*

    • Completes comprehensive individual nursing log, on an ongoing basis, for all individuals residing in the Agency's supervised living homes; compiles quarterly reporting.
    • Completes master physician orders and integrates with individual identification sheets; may assist with securing health care provider signatures.
    • Completes monthly medication and treatment sheets for all individuals assigned to caseload.
    • Completes quality assurance reviews to ensure Agency compliance with Department of Public Health (DPH) Medication Administration Program (MAP) requirements.
    • Completes quality assurance reviews specific to each individual's medical care and corresponding medical record documentation.
    • Provides training to Program Managers, Residential Supervisors and Direct Support Professionals specific to the medical and therapeutic needs of the individuals assigned to caseload.
    • Provides consultative support to Residential Supervisors regarding the changing needs of individuals.
    • Participates in hospital admission and discharge planning.
    • Assists Residential Supervisors in securing community based services on behalf of individuals, as needed, such as Visiting Nurses Association (VNA) and hospice.
    • May provide specific treatments as ordered by primary care physicians including, but not limited to, g-tube feeding and medication administration MAP Certified staff are not authorized or trained to do.
    • Provides initial MAP training to Agency staff utilizing the DPH approved curriculum.
    • Provides recertification training and testing for Agency staff on an ongoing basis to ensure staff maintain appropriate certifications.
    • Provides rotational emergency on-call nursing support, as approved, responding to individual medical crises off hours as deemed necessary by the manager on-call.
    • Participates on the Agency's clinical review team assessing medication occurrences, as well as, psychotropic medication assessment and treatment plans.
    • Assists with clarification of orders for staff adherence according to MAP.
    • Promotes compliance with Massachusetts Department of Developmental Services (DDS) health screening recommendations.
    • Promotes understanding and compliance with medical/clinical instructions.
    • Communicates with and facilitates communication between health care practitioners, staff, and family.
    • Advocates for the optimal medical care of all individuals served.
    • Serves as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC).

    PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

    • Valid Registered Nursing License in the state of Massachusetts.
    • Minimum 2 years prior experience working in a medical setting as a registered nurse.
    • Valid driver's license in the state of Massachusetts.
    • Demonstrated experience in community health nursing, visiting nursing and/or nursing specific to individuals with unique developmental or traumatic brain injury challenges.
    • Successful completion of all state and Agency mandated trainings.
    • Ability to complete state mandated certification program to teach coursework developed by the Department of Public Health (DPH) Medication Administration Program.
    • Ability to travel to residential homes throughout Cape Cod to provide medical clinical assessments.
    • Ability to provide hands-on personal care.

    SUPERVISORY RESPONSIBILITY

    • None.

    WORKING CONDITIONS/PHYSICAL DEMANDS

    • Frequent standing, walking and bending.
    • Ability to lift up to 50 pounds to transfer individuals using hydraulic equipment.
