    Overview


    Fairview Pharmacy Services is hiring a full-time Clinical Pharmacist to provide clinical support and consultative services in a complex outpatient/infusion pharmacy operation at the M Health Clinic & Surgery Center, blocks from the University of Minnesota campus.

    This position partners with key stakeholders to drive medication safety, customer service, process improvement, clinical and formulary programs and initiatives and regulatory compliance.

    The Oncology Pharmacist will act as a primary resource in the development and education of students, residents, staff and interdisciplinary members of the care team.

    This is a great opportunity for a Pharmacist who has completed a PGY1 residency or a Pharmacist who has at least one year of experience in a complex pharmacy.

    Responsibilities/Job Description


    Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.

    Act as resource for pharmaceutical care to patients.

    This includes partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives.


    Provide clinical pharmacy services to all patients in assigned area including completing review for all patients daily, monitoring, adjusting patient therapies and documenting changes according to department policies.

    Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.

    Effectively consult and educate patients, staff, and students on drug

    Execute the function of staff pharmacist

    Other duties as assigned

    Qualifications

    One year pharmacist experience in complex pharmacy operation or completion of an ASHP-accredited PGY1 residency program

    Licensed or eligible for licensure as a pharmacist in Minnesota
