- Respond to referrals and tasks from team members promptly
- Complete accurate medication reconciliations
- Recognize and address medication adherence gaps
- Submit and complete prior authorizations for medications
- Assist patients with medication access and address cost issues
- Assist the pharmacist with daily operations as needed
- Participates in HEDIS and quality initiatives
- Perform other duties as assigned
- Outreach to patients to recognize and address pharmacy needs
- 2+ years' experience in a healthcare environment
- Unrestricted license in state of practice
- Experience working with an interdisciplinary team
- Experience with electronic medical records
- Medicare and Medicaid experience preferred
- Works and communicates well in a team environment
- Excellent communication skills
- Highly organized and detail-oriented
- National pharmacy technician certification
- High School Diploma or GED
- Experience and knowledge of basic technology
- An entrepreneurial spirit. Must be a tenacious self-starter
- Flexible and adaptable to a constantly changing workload
- Must enjoy working in a fast-paced environment
- A sense of humor and a down-to-earth nature
