    Hospice - Home Care Aide

    Summary:
    • Provide direct personal hygiene and care to the Home Care/Hospice patient according to written plan of care
    • Document all patient encounters and care according to policy
    • Demonstrates and encourages team behavior and exceptional patient/guest experiences.
    • Upholds and promotes patient safety and quality.
    Qualifications

    Minimum Knowledge, Skills, Experience Required

    Education:

    High school Graduate of GED. Must have certification as Certified Home Health Aide

    Experience:

    One year of health care experience preferred

    Additional Skills / Abilities:

    Good communication and organizational skills
