    John BerchmansJOB SUMMARY:


    The Service Coordinator is responsible for basic case management and day to day coordination of supportive service activities performed on behalf of the residents of the property under the direction of the Director of Community Managers.

    Must have the professional knowledge and skills sufficient to assess the physical needs of residents, advocate on their behalf and refer for services as observed and/or requested.

    It requires the ability to document services and complete HUD and other reports as required.


    ESSENTIAL FUNCTIONS:
    Manages the day to day services of assigned cases.
    Must be able to manage a varied number of cases. Monitors the delivery of services to residents.
    Reports any suspected abuse to the appropriate agency.
    Advocates and negotiates on behalf of residents as neededSecures resident files to ensure confidentiality.
    Must obtain a signed confidentiality release from resident before disclosure.
    Makes appropriate referrals for resident services as requested.
    Works as part of a team, as well as complete assignments independently.
    Implements policies and procedures in compliance with agency, funding, licensing, and certification.
    Engages in effective communication with all levels of staff, state personnel, advisory board, community groups, clients and volunteers. Liaison between community agencies, service providers, and residents.
    Sets up volunteer support programs with service organizations in the community.
    Develops and maintains a current directory of community-based service providers. Participates in long and short range planning for the department of service coordination.
    Must be able to work in a fast-paced & ministry oriented environment. Maintains a professional, compassionate and welcoming attitude toward managers, co-workers, residents, and visitors.
    Other duties as assigned.


    PHYSICAL DEMANDS:
    Must be able to stand and walk or sit alternatively depending on specific needs of the day. Estimate 60% of time is spent on feet and 40% sitting at desk.


    MINIMUM QUALIFICATIONS:
    Must be available evenings, nights and weekends for emergencies.

    Must have the following computer skills:
    Microsoft Word, Excel, Outlook, Yardi and Adobe. Effective written and verbal communication skills.
    Experience working with the elderly is desirable.


    EDUCATION:


    Bachelor's of Social Work or Bachelor's in Social Services and a minimum of one (1) year experience working with the elderly, preferred.

    A minimum of three (3) years experience working with the elderly and disabled, in a professional work setting, may be accepted in lieu of degree.
