    Key Responsibilities
    Partner with residents and care teams to deliver physical therapy aimed at maximizing residents' functional abilities and independence.
    Conduct evaluations and develop personalized treatment plans including short- and long-term goals, therapy modalities, and care frequency.
    Collaborate with residents, caregivers, and healthcare team members to ensure comprehensive, resident-centered care.
    Monitor progress, adjust treatment plans as necessary, and document all interactions accurately in the Electronic Medical Record.
    Educate residents and caregivers on assistive devices and therapeutic exercises, ensuring proper use and understanding.
    Communicate with physicians and team members about treatment plans and discharge needs.
    Supervise and mentor assistant-level staff, ensuring adherence to professional standards and regulations.
    Maintain department cleanliness, equipment, and compliance with infection control procedures.
    Stay current with professional development through memberships, publications, and in-service education.
    Assist in developing new programs and services within the rehabilitation department.
    Required Qualifications
    Active license as a Physical Therapist in the applicable state.
    Bachelor's degree in a related field; Master's or Doctorate preferred.
    Basic Life Support (BLS) certification preferred.
    Prior experience in long-term care or skilled nursing facility preferred.
    Strong communication, documentation, and interpersonal skills.
    Proficiency in Microsoft Office.
    Preferred Experience / Skills
    Experience working in elder care or rehabilitation settings.
    Knowledge of Medicare, Medicaid, and private insurance reimbursement regulations.
    Ability to work collaboratively within an interdisciplinary team.
