    SummaryReporting to the Chief Operating Officer, Population Health is accountable for the overall population health strategic direction for the NEA Baptist Clinic (NEABC).

    This leadership role will work directly with the Administrative Team in the development of advocacy strategies, partnerships as well as policy and program options that support the population health strategic direction.

    The Director will support the development and implementation of communications, partnerships and actions on key population health issues that are intended to improve the overall health of the population and reduce health inequities.

    The Director will develop partnerships with other decision-making bodies in the community and with senior levels of Government, the private sector, other health authorities and multi-sectoral community groups, in order to influence policy to improve health status.

    In addition to this strategic policy role, the Director will initiate and manage specific strategic programming within NEABC aimed at improving population health and will work closely with the CMO, Operational Directors, Managers, and others who are engaged in NEABC program delivery to ensure that NEABC strategies, programs and actions are aligned with NEABC's population health strategic direction.

    A primary focus will be to monitor provider performance metrics and work with the practices on opportunities to engage, improve, and adhere to practice transformation models.

    ResponsibilitiesDevelops coordinates and evaluates a NEABC wide population health strategy that incorporates leadership, partnership, policy development and advocacy to address the social determinants of health.

    Leads the process with NEABC Admin Team to develop partnerships with the Community and other groups, in the development of healthy public policy and programs that address population health issues.

    Undertakes the analysis of population health status and population health determinants and develops system wide strategies that improve population health and reduce health inequities including developing policy and program proposals and business cases for NEABC initiatives.

    In collaboration with other NEABC staff, liaises with organizations, agencies and community services to identify present, and future population health issues and trends and initiate action by NEABC partners to address the social determinants of health.

    Oversees NEABC's community health promotion grant programs, which build community capacity to improve health among specific populations within the community.

    Develops and supports partnerships with researchers to enhance the knowledge base on population health interventions.

    Provides education and support to NEABC teams in order to develop the competencies of NEABC staff to undertake population health initiatives.

    Leads the development, reporting, monitoring and analysis of population health performance indicators.

    Works in patient care as a Nurse Practitioner as needed in order to the close care gaps needed to meet quality metrics.

    Requirements, Preferences and Experience Master's or DNP degree for Advanced Practice Registered Nurse Proficient in medical terminology and insurance Five years' experience in quality improvement or related experience.

    Healthcare industry experience preferred.
