Where You'll Work
At CHI Health Midlands we proudly serve Sarpy County and surrounding areas in Nebraska with patient-focused care. When designing our facility, we kept the patient viewpoint in mind, setting a tone for a more comfortable, less stressful hospital stay. We provide our patients with some of the highest quality scores in the country in heart and vascular, emergency, and surgical care. Also, we offer high quality care in diagnostics/radiology, women's care, home care, orthopedics, sleep and breathing disorders.
Job Summary and Responsibilities
Eligible for up to a $5,000 Sign-On Bonus and up to $10,000 in Student Loan Assistance

You'll provide support, guidance, and resources, helping patients get the care they need while connecting them to the community services that make life easier.
What You'll Do:

Offer crisis support, counseling, and resource referrals for patients and families

Help plan safe discharges and transitions of care

Coordinate with healthcare teams to keep patients supported and informed

Connect patients to community resources for housing, food, transportation, and more

Job Requirements
Education: Master's Degree from a school of social work accredited by the Council of Social Work Education REQUIRED
License/Certification:Social Work Certification REQUIRED
